Physician referral program

We’re excited to offer a patient referral program. If you are a healthcare provider with a patient who would be a good fit for Blair Health, please refer them to us. We will follow up with your patient directly. Blair does not require a referral, so patients can also start on their own with the free assessment.

I confirm the patient has consented to this referral and understands that Blair Health is a private, non-publicly funded healthcare service.

I acknowledge that Blair Health is unable to coordinate routine cancer screening for patients (e.g., mammograms, HPV testing) and I will continue to organize this patient’s screening in the primary care setting.

Some menopausal concerns are not suitable for evaluation on a virtual care platform. I confirm my patient does not currently have abnormal uterine bleeding and/or postmenopausal bleeding. Should this start, I can work collaboratively with the Blair Health clinical team to ensure my patient gets the appropriate in-person care and assessments, when necessary.
