I acknowledge that Blair Health is unable to coordinate routine cancer screening for patients (e.g., mammograms, HPV testing) and I will continue to organize this patient's screening in the primary care setting.

Some menopausal concerns are not suitable for evaluation on a virtual care platform. I confirm my patient does not currently have abnormal uterine bleeding and/or postmenopausal bleeding. Should this start, I can work collaboratively with the Blair Health clinical team to ensure my patient gets the appropriate in-person care and assessments, when necessary.
